Handbooks & Forms

  • Recommendation Forms for Class of 2011:  Current juniors (rising seniors) are reminded to turn-in their Parent Comments & Observations Form and their Student Activities Sheet to their Guidance Counselor as soon as possible.  This will assist in the writing of the Guidance Counselor college letter of recommendation for the student.
  • Senior Surveys & Final Transcripts:  If you have not already done so, please forward Parts 1 and 2 of your Senior Survey forms to the Guidance Office along with a pre-stamped and pre-addressed envelope to the one college you plan to attend.  If you have questions, visit the Guidance Office.
  • Letting Go Night Presentation Online:  The PowerPoint presentation given by Dr. Sheryl Sawin from our Letting Go Night held on June 2, 2010 is available online:  Letting Go Night.
  • Rutgers Needs Your Grades and SAT’s:  Attention seniors!  If you applied to Rutgers but have not done your on-line self report of your final marks for grades 9, 10, and 11, you need to take care of that as soon as possible.  Rutgers no longer enters the grades from your MHS transcript.  Likewise, if you have not arranged for your SAT scores to be sent from College Board to Rutgers, you need to take care of that as well.  The Guidance Office staff no longer includes photocopies of SAT scores with transcripts that are sent out to colleges.  If you are not sure that you have addressed these matters, you can check on-line at the Rutgers website, or visit the Guidance Office.
  • NCAA Registration for Juniors & Seniors:  Student-athletes in the 11th grade who are hoping to play at the NCAA Division I or II levels in college are encouraged to register with the NCAA Eligibility Center.  All student-athletes must register with the NCAA Eligibility Center prior to making any official school visits or receiving a written offer of athletically related financial aid with “D-I” or “D-II” colleges.  You can register with the NCAA Eligibility Center at www.eligibilitycenter.org.  Senior athletes who have not already done so, should register immediately.  Visit the Guidance Office or see Mr. Rosa if you have questions.
  • SAT Question of the Day:  Are you signed up for the SAT Question of the Day?  If not, you can register to receive the question daily on your e-mail.  You will receive just one question each day.  After you answer it, you will receive a complete explanation as to whether your answer is correct or incorrect and as to the reason it was right or wrong.  Just think, at 365 days a year, if you start as a freshman, you will have done more than 1000 practice questions before you take your senior year SAT’s, and all for an investment of just two or three minutes a day.  And, it’s FREE!  To register to receive your official SAT  Question of the Day, visit www.collegeboard.com and click on Resources for Students.  While you are there, take a look at the four-month SAT prep. program that College Board offers for $69.95.
  • Peer Tutors:  If you need help in one or more of your classes, don't forget the Honor & Service Society's Student Tutoring Program.  If you need some help getting through a subject or bringing up a grade, peer tutors are available. See Ms. Church-Williams in B-107 or download the Honor and Service Society "Tutor Request Form".
  • SAT/PSAT:  Any student who requires special accommodations for SAT, PSAT, and AP tests must file a SSD Student Eligibility Form with the College Board. Any student who has not already done so must pick a form up from Mrs. Lehrer in the Guidance Office immediately.  For October PSAT testing, eligibility forms must be determined complete by September 9, 2009.

Be sure to submit the form by the appropriate date for the test you wish to take. For exact dates and deadlines, visit the College Board's Important SSD (Services for Students with Disabilities) Dates 2009-2010 Academic Year page.

The College Board recommends that for the PSAT/NMSQT you submit your completed eligibility form by June 15th in order to guarantee that you receive the appropriate practice materials. For all other College Board test dates, use the Regular Registration Postmark Deadline as the deadline for the accommodation form. Forms are available in the Counseling Office. For more information regarding Services for Students with Disabilities visit College Board Online's Services for Students with Disabilities (SSD) page.  You may also call 609-771-7137.
