Having trouble getting a sports physical done...
Premier Urgent Care

No appointment necessary
Premier Urgent Care is a full service medical center, which means that in addition to sports physicals, they offer a full range of medical services ~ X-Ray, EKG and full lab diagnostics

Please make sure to print, fill out and complete all sports physical forms as soon as possible. These can be optained on our website, Forms, Sports Physicals.
The yellow card stock Co-Curricular Advised Consent Form is needed to complete the Fall Sport Packet. These are located in the Main Office near the Sports Physical Tray. When you go to drop off your filled out paperwork, please add this to your packet of information.
Please place ONLY completed packets in the tray for processing.
